Documents Required for Screening
Applicants must bring clear copies of the following applicable documents to the screening session. Applicants who fail to provide these documents on the day of screening will not be permitted to test.

copy
of your birth certificate or a certified letter of birth verification from the courthouse (minimum of 18 years of age required). - A
copy
of citizenship verification if you are a naturalized citizen of the United States. If you are not a citizen or a naturalized citizen, you must produce a permanent resident card. If you are a citizen, THIS DOES NOT APPLY TO YOU. - A
copy
of your high school diploma of G.E.D. Equivalency certificate. - You must produce all discharge certificates (DD214 Member 4 Edition or NGB22) from all branches of the military. All discharges must contain the type of discharge, character of service, years of service, and rank upon discharge.
Also, if you are currently serving in the National Guard or Reserves, you must produce your DD214 and a letter on military letterhead signed by a commanding officer with the following information:
- date of entry
- estimated time of release
- current rank
- character of service, i.e. honorably serving
- A
copy
of your Selective Service System registration card or other proof of registration. (Males between 18 and 26 years of age should contact (847) 688-6888, Federal Selective Service, or https://www.sss.gov/RegVer/wfVerification.aspx to verify registration.) - A
copy
of college transcripts or diploma if applicable. - A
copy
of a photo I.D. - A
copy
AND the original of a valid driver's license.
Please ensure that you bring complete address and zip code information for all previous employers. These addresses will be used to complete employment reference forms that will be distributed to you on the day of screening.
You will be required to complete additional documentation at the screening. You can obtain copies of some of these forms (CMS 100, Applicant Information Sheet, and Release of Information) online at http://www.illinois.gov/idoc/Pages/Careers_At_IDOC.aspx. Applicants are strongly encouraged to print and complete these documents prior to screening and bring the completed forms to the screening session.
